The successful candidate will be responsible for the day-to-day implementation, coordination, and successful delivery of nutrition and health project activities throughout the project lifecycle. The role involves providing oversight and supervision of health facilities managed by State and Local Government Authorities (LGAs) and supported by ACF, strengthening the capacity of Ministry of Health (MoH) healthcare workers through training and mentorship, and ensuring timely monitoring, reporting, and continuous updates on project progress and outcomes.

Key Responsibilities :
Mission 1: Nutrition and Health situation monitoring The Nutrition and Health officer is responsible for- Mapping of Nutrition and health activities;
- Technical support for community level activities;
- LGA Monitoring and evaluation of nutrition and health interventions.
Responsibilities:
- Update and maintain the 5W/4W mapping (Health and Nutrition sector) tool monthly for the operational LGA, and share with the Nutrition and Health Programme Manager;
- Update tracking tool to monitor programme screening-based data on acute malnutrition; collect health facility-based disease surveillance reports and update morbidity reporting tool for assigned health facilities/LGAs;
- Participate with the M&E team in the supervision of data collection for nutrition and health assessments;
- Liaise with nutrition and health assistants for accessing the community support group and health facility database regularly and updating the overall health facility-based activities;
- Conduct ongoing capacity assessment of health workers on CMAM service delivery, IMCI service delivery, reproductive health service delivery, vaccination, etc., and review the training needs and/or coaching needs;
- Conduct training and establish tom brown centers in the health facility catchment areas for the management of moderate acute malnutrition
- Support the Nutrition and Health Programme Manager in facilitating and organizing technical trainings for health workers (including on LMIS, HMIS);
- Ensure use of predefined quality assessment checklist, on-the-job coaching guidance, as well as support for Nutrition and Health assistants in using these tools;
- Review financial, logistical, and HR needs on a monthly & quarterly basis and share with the N&HPM;
- Ensure that all health facilities/sites have adequate supplies to deliver uninterrupted health care services through identifying needs, informing the line manager regularly;
- Support the Nutrition and Health Programme Manager in organizing the LGA-based coordination meeting;
- Prepare an individual monthly action plan in reference to the work plan and share with the Nutrition and Health Programme manager in the regular monthly planning meeting with nutrition and health teams led by the Nutrition and Health Programme Manager.
Mission 2: Supervise LGA and facility level activities in the framework of health and nutritional services
The Nutrition and Health officer is responsible for- Supporting the programme manager in management of the project cycle in nutrition and health
- Ensuring projects are implemented in line with donor proposals and requirements and in accordance with Action against Hunger, Country and international standards.
Responsibilities:
- Ensure that all children and patients visiting ACF-supported health facilities are properly examined and treated according to national health and nutritional protocols and standards;
- Identify gaps for WASH in health facilities and inform the Nutrition and Health Programme manager regularly;
- Ensure availability of essential nutrition & health supplies in assigned health facilities, including data tools;
- Monitor for appropriate documentation–IMCI and CMAM treatment registers/tools, individual child growth monitoring cards, ANC/PNC cards and delivery registers;
- Identify financial requirements, logistic needs, and HR support in reference to the work plan and inform the NHSM to prepare a budget forecast;
- Draft supply request and share with Deputy Nutrition & Health Manager monthly;
- Facilitate quarterly review meetings/workshops with respective government health managers at the State/LGA level;
- Develop a monthly monitoring and supervision plan for the LGA and share it with the Nutrition and Health Programme Manager;
- Ensure that specific monitoring findings are reported to the Nutrition and Health Programme Manager with potential solutions;
- During the supervision visit, identify gaps in the supply side and report to the Nutrition and Health Programme Manager for remedial action;
- Ensure that referrals are made to the nearest higher-level centre and tracked for high-risk pregnancies, SAM with medical complications, and other conditions requiring higher-level care.
Mission 3: Training and managing the programme teams (nutrition and health assistants)
The Nutrition and Health officer is responsible for:- The twice yearly assessment of the skills and performance of the Nutrition and Health Assistants
- The management of his/her teams (leadership, monitoring, motivation
Responsibilities:
- Maintain an individual monitoring & supervision plan for the supervisee;
- Regularly follow up with the teams on the progress of the activity, challenges, and guide necessary actions;
- Share training plans with identified needs with the PM well in advance to facilitate support from HR/Admin/Logistics;
- Track Supervisee’s daily attendance, validate monthly attendance for supervisee, and submit it on a timely basis to HR;
- Communicate with HR for the necessary staff meeting, performance review, appraisals, and leaves of the supervisee;
- Liaise with HR to provide orientation on ACF charter/core values/child safeguarding policy/staff code of conduct, etc;
- Ensure rational use of all medical equipment and stationery in health facilities;
- Ensure that Nutrition and Health Assistants are using the standard monitoring checklist during visits to health facilities.
Mission 4: Information management, reporting and coordination
The nutrition and health officer is responsible for:- Ensuring that regular reports are provided by health workers in supported health facilities
- Support ing the LGA in consolidating quality and time facility data
- Reviewing and analysing facility service delivery data and providing feedback to LGA and health facilities
- Facilitating and jointly planning LGA level coordination meetings
Responsibilities
- Maintain and update the database for health workers, community health workers/volunteers, trainings, and share with the line manager;
- Prepare a consolidated tracking tool for supportive supervision visits/on-job coaching sessions and share with the line manager;
- Update LGA work plans, individual work plans, and share with the line manager;
- Use the appropriate attendance sheet for on-the-job coaching sessions, workshops/meetings, and training, and share a copy with the line manager;
- Use a specific supervision checklist during supervision visits and archive filled supervision checklists with a summary report – every month;
- Fill the health evolution reports every month for the LGA and share them with the line manager for consolidation;
- Support government health workers in preparing the integrated diseases surveillance report, monthly facility-based DHIS2 report;
- Provide a summary analysis from the LGA report, including:
- Significant contextual changes that have an impact on nutrition & Health operations.
- Bullet points on progress of training, on-the-job coaching, and supportive supervision;
- Feedback on quality of service provision through monitoring visits (checklist to be used);
- Key challenges and proposed solutions
- Collect consumption reports from health facilities, compile for LGA, and share with the line manager with needs for the upcoming month;
- Regularly report and coordinate with line management
Position Requirements:
- Bachelor’s degree in Nursing, Midwifery, Public Health or other relevant qualifications with experience in Public Health and CMAM programming;
- Minimum of three (3) years’ experience in primary health care program;
- An excellent report-writing skill is required, including MS Word, Excel, and PowerPoint;
- Fluent in written and spoken English and Hausa;
- Good communication and Interpersonal skills;
- Team management experience with similar programs;
- Ability to observe and to provide feedback on the activities;
- Ability to manage staff according to local culture and organization policies.
